Omegalife Hospice Services

The OmegaLife Hospice Team

delivers an oriented approach to providing expert medical care, pain management and emotional and spiritual support for patients and family. Our Team of Caring Professionals includes the following:

Primary Care Physician

is responsible for overseeing and approving the patient’s Plan of Care.

Hospice Medical Director

approves the patient’s Plan of Care and may also serve as the patient’s attending physician.

Registered Nurse

serves as the patient’s Case Manager. Coordinates care by making home visits to monitor the patient, teach caregivers, provide skilled care, comfort and support. Make recommendations to the attending physician.

Certified Nursing Assistant

is responsible for providing assistance with the patient’s personal care, under the supervision of the Hospice RN.

Social Worker

assists in providing emotional care to patient and family members. Addresses financial issues. Help obtain any governmental support. Assists with end of life preparations such as funeral and mortuary arrangements.

Hospice Chaplain

spiritual support to all patients and family members in conjunction with the patient’s clergy. Also available to conduct or attend memorial services.

Volunteers

spend time with the patient, offer companionship and provide relief for the family.

Grief Support

comprehensive bereavement services are offered to family members dealing with loss.

Dietitian

will be consulted as needed to improve the patient’s nutritional status.

Speech, Physical and Occupational Therapists

will be utilized as needed to improve the patient’s quality of life.

Provider

spend time with patients or their families to offer companionship and provide relief to the family.

Aessa Bates, MSN, APRN, AGACNP-BC

Administrator. Director of Clinical Services

Aessa Bates is a board-certified Adult-Gerontology Acute Care Nurse Practitioner with over 15 years of nursing experience. She began her career at the bedside, where she developed a deep commitment to compassionate, high-quality care before transitioning into advanced practice.

Aessa has spent much of her career serving the geriatric population in both acute and home based settings. She is known for her empathetic, patient-focused approach and is dedicated to supporting patients and families with dignity and compassion during life’s most vulnerable moments.

A recognized leader and mentor, Aessa is committed to delivering compassionate, high-quality care and empowering nurses to uphold the same standard – where ever care is provided.

She has been honored as one of Houston’s Top 100 Nurses and is a recipient of the Good Samaritan Award.

Dr. Mohammad Siddiqi

Medical Director

Dr. Mohammad Siddiqi completed his Doctor of Medicine at the University of Missouri, Columbia in 1995. He has been a member of the American Board of Internal Medicine since 2000.

Dr. Siddiqi has been the Medical Director of OmegaLife Hospice of Texas, Inc. since 2016. He has since been an invaluable asset to the agency.

Dr. Siddiqi has more than 19 years of extensive hospice experience which equipped him to provide the agency’s patients and their families with the highest quality of hospice care. He sees patients for face-to-face visits in the Greater Houston area and the Dallas-Fort Worth area. He also provides medical expertise and guidance to the hospice interdisciplinary team, including nurses, social workers, hospice aides, volunteers, and chaplains. He plays a crucial role in overseeing physician services and the medical components of the agency, and in ensuring that patients are fully informed about their care options and that their choices are respected, and that their dignity is maintained throughout their hospice journey.

Dr. Siddiqi is well respected by his peers, the clinical team, his patients and their families for his compassion, skills, and knowledge, and for making such great impact in their lives.
